This tournament will raise funds for the Young Adult Diabetes Program at Women’s College Hospital in memory of Hanna Valle, daughter and sister of tournament co-founders Frank, Max & Debbie Valle. Hanna was a curious, thoughtful, sensitive and spirited young woman. Hanna left a lasting impression on many people and although she left us far too soon, her bright light continues to shine. Through Hanna’s journey with diabetes, the family experienced firsthand the potential of this program. They also recognized the impact the program could have on more young adults with 100% of the proceeds being generated from this golf tournament supporting the program. They hope to provide more people with the opportunity to experience the same care Hanna did. The White Butterfly Charity Golf Tournament is a wonderful way to honour Hanna’s spirit and the Young Adult Diabetes Program at Women’s College Hospital. The Young Adult Diabetes program is dedicated to assisting young adults with diabetes make a smooth transition to an adult diabetes clinic from their paediatric providers. The multidisciplinary diabetes team specializes in complex type 1 diabetes care for people 18-25 years old and works in partnership with paediatric endocrinology treatment centres to help improve the transition. The Young Adult Program is also supported by a Nurse Practitioner who functions as a transition navigator to assist young people in achieving their self-management goals and reduce patients becoming lost to follow up. Through this program, patients also have access to a social worker and dietitian at each appointment. The team provides counseling on living independently with diabetes, travelling, stress management, and how to navigate competing priorities on a young person’s time.
